Clinical Assessment and Monitoring

Quality Home Care Solutions addresses each client’s needs on an individualized basis. Not only do we strive to provide compassionate and experienced caregivers, but we also understand that every individual’s needs differ. Therefore, clinical assessment and monitoring is part of our process in order to provide the best in high-quality care services for our clients.

Our clinical assessment helps us determine your needs, as well as your qualification for services. Due to new healthcare regulations, prospective and/or existing clients must show dependence (meaning you need physical, hands-on assistance, you cueing or supervision or help every time you complete one of the below-listed activities). You’ll need to qualify or show dependence in at least two activities of daily living (ADL’s). For example, common activities of daily living include:

Bathing      Dressing        Grooming        Mobility

Transferring      Positioning     Eating       Toileting

For example, many seniors don’t bathe on a daily basis, but when they do, they need help getting into or out of the tub. The same goes for getting into or out of a wheelchair, mobility within the home environment, or dressing and grooming, as is often the case with those diagnosed with some form of dementia, paralysis, injury or post-surgical rehabilitation. Safety is of primary importance – to the client and his or her caregiver in such scenarios.

A variety of questions are asked during the assessment in order to determine eligibility. Not only does our clinical assessment determine specific need and focus, but monitoring your loved one’s condition over time. For some, conditions worsen or abilities decrease, and such assessments help us continually provide the best in safe, quality based services to meet the needs of you or a loved one.
